Momiji@Home provides access to integrated and individualized care that supports seniors to live healthier at home, helps reduce emergency hospital visits, and promotes general well being and physical health.

Momiji@Home PLUS (“PLUS”) is an expanded program of the fee-based service Momiji@Home(“At Home”), designed to enhance the quality of life for seniors living independently in the community—particularly Japanese Canadian seniors. Grounded in Momiji’s mission to “support aging in place,” this program promotes health, independence, and social connection while providing individualized and culturally appropriate services.
A key feature of PLUS is that, unlike the traditional At Home service (which requires a minimum 3-hour visit), it allows for shorter visit times and offers more specialized service options tailored to clients’ needs.
